With the HealthShare Form Builder, you can quickly design and instrument online forms using an intuitive graphical user interface. Forms you create in the Form Builder can be deployed within HealthShare products such as Personal Community and Provider Directory. 

Extensive online help is available in the Form Builder to help you get started. You do not need to know special coding syntax to design simple forms.
